In line with the implementation of the TES-Tulong Dunong Program (TDP-TES) handled by the UniFAST Secretariat. Below is the APPROVED Masterlist of TDP-TES grantees for 2nd Semester AY 2020-2021 for billing. Please prepare the following documents relative to the list attached:

1. Certified True Copy of Registration form(2nd semester2020-2021) – ORIGINAL
2. Certified True Copy of Grades (1st semester2020-2021)- ORIGINAL

GROUP REQUEST for the aforementioned documentary requirements was forwarded to the Registrar’s Office(NO NEED FOR INDIVIDUAL REQUEST). Please PRESENT the following requirements to the Registrar’s Office to facilitate the release of CTC of COR and Copy of Grades:

1. Official Receipt(payment for the requested documents)
2. Two photocopies of 2nd Sem 2020-2021 Certificate of Registration(COR)
3. Screenshot of 1st Semester 2020-2021 SIS Grades

Please submit the hard copy of the documents to Campus Scholarship Office – c/o Ma’am Emy on or before February 15, 2022.

*HARD COPY OF DOCUMENTARY REQUIREMENTS ARE TO BE SUBMITTED TO CHED REGIONAL OFFICE IV, 2ND FLOOR, C.P. GARCIA AVENUE, UP DILIMAN, QUEZON CITY.
