FRESHMEN ENROLLMENT ADVISORY No. 2

Admission & Registration Services

  • Confirmation via link provided in the PUP Santa Rosa CAEPUP Qualifier’s Notification received via email.

  • Submission of Required Credentials:
    Qualifiers must submit the original copy and two(2) photocopies of the following documents to the Admission and Registration Services Section on or before October 30, 2021 personally or thru courier:

    Admission and Registration Office
    Polytechnic University of the Philippines – Santa Rosa Campus
    LCA Blvd., Brgy. Tagapo, City of Santa Rosa, Laguna


    a. PUP Santa Rosa CAEPUP Qualifiers Notification printed document (sent via e-mail)
    b. Grades 10 and 11 Certified True Copy of Grades with school dry seal and signature
    c. Original Senior High School Card (F138) with school dry seal
    d. Certificate of Completion for Senior High School or Certified True Copy of Diploma with school dry seal and signature
    e. Original PSA Birth Certificate
    f. Certification of Good Moral issued by the SHS Principal/or Guidance Counselor with school dry seal
    g. Two (2) pcs of 2×2 inches colored picture with white background and with the applicant’s name on the photo.
    h. Two (2) pcs. long brown envelope.
    i. Two (2) pcs. long expandable brown envelope.
    j. Duly signed/notarized Certification Under Oath. (included in the received confirmation notification
    )

    For Senior High School Graduates/High School Graduates of Past School Year/s In addition to the above admission requirements:

    k. Certification from the respective senior high school registrar with school dry seal and noted by the Principal that no copy of F137A has been sent to other college or university.
    l. Notarized certification of non-enrollment in any technical/diploma/degree program immediately after graduation with “waiver” that if there is concealment of previous enrollment, PUP enrollment becomes null and void.


  • Online Enrollment(Automatically upon verification of submitted documentary requirements)
    Wait for the link to be send via email about the procedure on how to access your PUP Student Information System(SIS) account on or before the start of 1st Semester SY 2021-2022( October 11, 2021) 

    Additional requirements to be submitted online:
    m. PUP Medical Health Information Sheet for Students (to be uploaded soon on PUP Santa Rosa Blog Site). Do not leave any item unanswered and send to medical.starosa@pup.edu.ph
