HERE IS THE UPDATED LIST OF REQUIREMENTS FOR CLEARANCE IN ADMISSION AND REGISTRATION OFFICE(ARO).

The following should be provided individually:

• one(1) Expandable Envelope with name and course handwritten at the back, upper-left corner and at the center of envelope’s outside seal flap (use Pentel Pen) Placed inside expandable envelop were the following:

(Should be printed in long (8.5″x13″)bond paper, and provide 2 photocopies each)

1. General Clearance (signatures from concerned offices should be completed)
2. Certificate of Candidacy
3. NSTP form
4. Personal Directory Information
5. Photocopy of all Graduation Fee Receipts
6. PUP I.D. (kindly remove from the case)
7. 3 pcs. Documentary Stamps (for TOR, Diploma, and Certificate of Graduation)
8. 1 pcs. Ordinary Long Brown Envelope
   1. One large, brown ordinary envelope for TOR with name and course handwritten at the back, upper-left corner and at the center of envelope’s outside seal flap (use Pentel Pen)
   1. One small, brown ordinary envelope for diploma with name and course handwritten at the back, upper-left corner and at the center of envelope’s outside seal flap (use Pentel Pen)
